Laboratory and X-ray tests
  • Laboratory tests can be done at the hospital
  • Pregnancy and some urine tests will be done in the clinic
  • Ultrasounds and X-rays are done at the hospital
  • You are advised to book ahead for specialized X-ray and all ultrasound tests
  • Results can take up to one week to return, and you will be called only if the results are abnormal or problematic
  • Results will only be given to the patient
Transfer of Patient Records
  • The transfer of patient records to or from Hillcrest Medical Associates requires your written permission
  • Should this become necessary, our receptionist will provide you with the necessary forms
  • Please note that there may be a charge for the chart transfer, which would be the responsibility of the patient
Health Information Act
  • The information that you are asked to give is collected, used and disclosed in accordance with the provisions of the Health Information Act (HIA)
  • The access and privacy provisions of this legislation require that the Hillcrest Medical Associates protect your personal health information from unauthorized access, collection, use, disclosure or destruction.
